Beth Israel Deaconess Hospital-Plymouth (BID-Plymouth) was recently awarded a $245,818 Phase I grant from the Commonwealth of Massachusetts’ Health Policy Commission (HPC) to enhance the delivery of efficient, effective healthcare in the region BID-Plymouth serves. The grant is from Phase I of the HPC’s Community Hospital Acceleration, Revitalization, and Transformation (CHART) Investment Program.

BID-Plymouth will use the CHART grant to expand the efforts of the hospital and the Jordan Community Accountable Care Organization to improve care for more complex patients, including dually-eligible patients, to ensure that they receive personalized care and follow-up treatment within their home and community. The funding will also help BID-Plymouth develop programs to reduce hospital readmissions for these populations by focusing on better coordination of care between the hospital, the patient, and skilled nursing facilities.

“Through the CHART grant, the Commonwealth of Massachusetts recognizes that BID-Plymouth and our Accountable Care Organization are working to improve the health and well-being of our patients,” says Dr. James Fanale, senior vice president of System Development, BID-Plymouth. “Now we can focus on enhancing this delivery of care to include more complex patients who deserve a better, more coordinated approach to their care.”

BID-Plymouth plans to submit a CHART Phase II grant application this spring for an additional grant to further develop and expand existing healthcare programs and services in the region.
