The Onset Foursquare Church, located at 301 Onset Ave. in Onset, will be accepting Salvation Army applications for Christmas assistance on Nov. 7, 14 and 28 from 10 a.m. to 2 p.m. and on Dec. 1 from 5 to 8 p.m.

The following documentation is required for food assistance and toys for children up to age 16:

If receiving welfare, social security, SSI or disability: Original photo ID for adults, original Massachusetts health card and original birth certificates for all children in the home. Official rent receipt or bill with name and current address dated within the last two months for all adults.

If employed or receiving unemployment: original Photo ID for adults, two recent pay stubs or unemployment stubs, original Massachusetts health cards and original birth certificates for all children in the home. Official rent receipt or bill with name and current address dated within the last two months for all adults.

If more than one family lives in one household, each family should apply separately. Anyone 18 or older wishing to apply separately will need to provide photo ID, proof of income and proof of residence.

Anyone with foster children in their household should bring the child’s passport.
